EyeRise is a digital wellness software platform, not a healthcare provider. Some clinic-related workflows may involve patient contact or attribution information. EyeRise is designed to minimize sensitive data collection and to support responsible privacy practices. Where required, additional clinic agreements or privacy terms may apply.

What EyeRise may collect
Depending on how you use EyeRise, we may collect:
- Full name
- Email address
- Phone number
- Physical or home address (for example when you complete a clinic One Scan setup form)
- Clinic or partner attribution (practice recommendation code, partner name, setup token context)
- Consent records (version, timestamp, and the consent text you agreed to)
- Device and app usage data needed to operate EyeRise (device identifiers, app version, platform, settings sync)
- Payment and subscription metadata through Stripe (we do not store full card numbers)
- Support and communication records when you contact us
What One Scan does not intentionally collect
The One Scan clinic intake form is for wellness setup only. EyeRise does not intentionally collect diagnoses, prescriptions, medical record numbers, insurance numbers, or clinical exam results through One Scan. Please do not submit sensitive medical information in that form.
How we use contact information
We use contact information to provide setup instructions, account and sign-in messages, customer support, service reminders, and product updates when you have agreed to receive them or when permitted by law. Email is the primary setup channel; SMS may be used only when configured, consented, and allowed by applicable law.
Clinic QR / One Scan
When you scan a clinic QR code and submit the form yourself, EyeRise receives the contact details you enter. We may share limited information with that clinic or partner for attribution, setup status, support, and partnership administration. Clinics do not receive your full payment card data.
Data on your device
EyeRise stores preferences and wellness settings on your device and may sync account-related settings when you sign in. We do not sell your browsing history. We do not use third-party advertising trackers in the core product experience.
Service providers
We use vendors to run EyeRise, such as hosting, email delivery, SMS (when enabled), payment processing (Stripe), analytics (Google Analytics 4, when enabled on marketing pages), and customer support tools. These providers process data on our instructions and only as needed to deliver the service.
Your choices
- Reply STOP to opt out of EyeRise SMS where applicable
- Use unsubscribe links in marketing emails
- Contact us to request access, correction, or deletion of personal information, subject to legal and operational requirements
